Program Duration *

The value selected is the number of days after the qualified disaster was declared that the program will accept applications. Best practices 30 days. Please note that any changes to the program duration will result in an amendment to the agreement, which incurs a US$150 fee.

Grant Amount *

Determine how much each grant recipient will be granted in USD. ($250 - $500 is most common; $1,000 is the maximum allowed)

Max Applications *

Estimate the maximum number of IRP applications expected. This number enables the program to be limited to ensure that grants do not exceed donations available. A minimum of 30 is recommended to minimize the cost of each application and to comply with regulations by avoiding the targeting of grantees.

Additional Information

If additional information is needed to assist with Charitable Class Verification beyond the applicant's birth year and the last four digits of their team member ID, please list the additional question(s) that should be included in the IRP application. Social Security Numbers should never be requested.

Form submissions are used to initiate the process of launching a new Immediate Response Program (IRP). Once submitted, the request will be reviewed, and an IRP may be launched based on the information provided.
